2022 year-end Membership Survey

Survey is only open to current CAFTCAD members. Your responses will be kept confidential and anonymous, but we will require your name and email address to verify your membership status.

CONTEST RULES: Every question on the survey must be completed in full to be entered into the draw for three grand prizes of $415 towards your Nobis purchase! Must be a CAFTCAD member in good standing to complete the survey and enter to win. Members can only complete the survey once. Nobis shopping credit cannot be combined with other offers (including the CAFTCAD member discount). Restrictions apply. Winners will be selected and contacted on Tuesday, January 10, 2023.
